  • I live in California and had a few questions about some of the questions the form was asking of me. 32. If you received, or if you expect to receive, any payments from an employer other than your regular salary, report the payment(s) below. -- My company gave me a 3-month severance package so how would I go about inputting that info? Like, would I put I got got laid off on 02/15/2009(under from date) to 3 months exactly from that date(which is 03/15/2009)? Also, it asks the amount so do I put in the amount of the package before or after taxes? 33. Provide your employment history for the past 18 months, including your very last employer. If you worked for a temporary agency, a labor contractor, an agent for actors, or an employer where wages are reported under a corporate name, your wages may have been reported under that employer name. You may want to refer to your check stub(s) or W-2(s) to obtain the name of your employer. Provide the following information for each employer you worked for in the past 18 months: 33a. Employer you worked for the longest time (select one) 33b. Employer name 33c. Beginning and ending dates of employment 33d. Wages earned 33e. How you were paid (specify hourly, weekly, monthly, annually, commission, or piece rate) Note: Failure to report all your employers, periods of employment and wages correctly may result in your benefits being delayed or denied. -- I didn't quite understand these questions. For one, I had worked under a temp agency and I don't get if I have to put that temp agency under the employer name or the actual company that the agency had me working for? -- Under wages earned($), once again is this before or after taxes? -- Under "How were you paid?" - I was paid bi-weekly but that wasn't an option (hourly, weekly, monthly, annually, commission, & piece rate). Can anyone give me more input on this section? Thanks for taking the time to read this and thank you even more for taking the time to respond.

    Generally unemployment starts after severance ends, so if you were laid off 2/15/2009 and received 90 days pay your unemployment benefits would generally begin 5/15/2009 and you would not tell them about the severance because that would be in the past. On your second question i'd say you tell them whomever paid you so my guess is that is the temp agency. Be sure to consult your local unemployment office http://www.unemploymentcalifornia.net to fact check these answers but I believe they are fairly accurate.
