Answer:
You can have medicaid and private insurance. However for medicaid to cover anything you have to go to a medicaid approved provider. However, if you can afford private insurance, then you probably would (should) not be covered by medicaid.

If you qualify for Medicaid, you can't AFFORD insurance to cover anything else. Also, since your doctor knows you have Medicaid, in most states, he's not allowed to charge you anything at all - meaning, he'll have to give you "free" care, up to your deductible amount, and waive your copayments. The thing about Medicaid - is that it gives a very unrealistic idea to people on it, about how insurance really works. People with insurance, have deductibles and copayments - and a monthly fee. You'd be paying $300 a month at least, PLUS the first $2,000 or so in all medical bills, before the insurance even starts kicking in. If you can afford that - ditch the Medicaid, and buy real insurance.

You cannot get separate insurance for only your doctor. You have three options: 1) You can get your own insurance for everything (including what medicaid covers). 2) You can pay your doctor using only your own money, without using any insurance. 3) You can stop using this doctor.

Some states allow private insurance with Medicaid and some dont. But if your doctor does not take Medicaid, it wont pay him anything. Medicaid is not insurance and insurance does not work that way. If you want to see the same doctor you have to buy health insurance on your own and not use Medicaid.

If you have medicaid how are you going to afford private insurance to pay for one Dr? I'd find another Dr if I were you who accepts it. I would imagine if medicaid found out you were paying for a private plan they'd cut your water off & have you just have your one private plan. You can't double dip.
