Go to a sports store and get a worm threader. It is a hollow metal tube on a handle, costs 3 bucks or so. You slide the worm onto the tube, and then put the hook in the tip and slide the worm onto the hook from the tube. You might also want to invest in a "magic worm blower" bottle. It is a little bottle with a syringe tip and you inflate your worm with it so the worm will float at feeding level. Works wonders.

i have caught many fish using a worm on just a plain hook or a lead head. i would also try a lindy rig draged slow across the bottom.
