NJ Unemployment extension?

My unemployment ran out and I got a new form in the mail with a yellow attached paper that asks for places I have seeked work? has anyone else gotten this yet and if so....what happens?... you mail it in the Friday before your week is up? and then what...? also lol this is so crazy but I'm a little OCD I made a mistake with the date and I crossed it out and rewrote it lol you think I'll have a problem? Real answers please I worked for a major company that went out of business, and I am back in school working towards a degree...

I recieve the same information just fill it out as you look for jobs. Its very easy just take your time.

I just got a form telling me I got an extension, and instructing me to check in via computer or phone as usual. I started getting checks again, and I suspect so will you. Don't be OCD, the crossout is not goijng to matter.
