Very simple. Call 1800 8291040 and ask the customer service rep to give you any information about your outstanding balance with the IRS. Of, simply file form then fax it to the IRS who will send you transcript about your last 5 years of tax's. http://www.irs.gov/individuals/article/0,,id=232168,00.html or visit any of the IRS office and ask for print out of your files for the last 5 years. http://www.irs.gov/app/officeLocator/index.jsp
