    Pwede mong tanungin ang dati mong employer or yung employer mo ngayon. Meron silang records nun at siguradong tinatago pa nila. Kailangan mo nga lang makiusap. Kung wala ka pang employer eversince or ayaw nila ibigay, kailangan mo ng magpunta sa SSS at dun humingi ng tulong. magdala ka ng 2 valid ID. Click mo lang ang link sa ibaba pra malalaman kung paano. Check out the source I provided. It's for the government website of the Social Security Administration. There will be a list on there of everything you will have to have to apply for a replacement card. You have to have several forms of identification for safety purposes.

Don't you mean SS(Social Security) card? If yes, just find your local Social Security office and make an appointment with them. Explain your situation and they will send you a new card, they might even give you a new number if identity theft is a big enough threat. Make sure you make an appointment though! Those guys are always busy.

You will have to go to a local branch with proper identification to verify your identity. If you still have questions you can use the SSS automated telephone service by calling the SSS IVRS at 917-7777 or call 920-6446 to 55 to speak to an SSS representative from 8:00 am to 5pm, Mondays to Fridays.
