Can I get a free shot record online?

I need to fax an aouthorization to get my sons shot record?

  • i lost the original ones.i called the office where he got them done,and they said they need me to send them an aouthorization....what do they mean by that?....what do i need to do?.

Do exactly what the first person told you. Just type up an authorization for the office to release your childs records. OR call the Dr.'s office and ask them if they have an authorization to release medical records already typed up and have them fax it to you and just sign it and fax it back. OR go by the office and sign the release there.

Do as the first person said here. Have you tried looking online? Most states have an immunization website that with specific info you can bring up the your sons immunization records. Check into it.
