How To Fill Green Card Application Form?

Please help. im 17 and i need to fill ou an application form for health benefits (health card)?

• you see, i moved out on my own to a different province and i need to fill out this application form for a new health card for this province, but here, it says: "occupation, employers name, employer's address, postal code, and work phone #" but im still 17 and work in a fast food for part-time and i still go to high school - so do i just put (N/A)? or do i put my part-time job there as my "occupation"? will they still accept my application for health card even if i don't have an occupation here?

  were it says put the employers name an all that just fill who u work for now cause that is ur job dosent matter what kinda job you have u put that in so employers name ud put like subway if thats who u work for then u put there adress the postal code an work number an u put part time job
