General passport application question?

Question about Passport application after oath ceremony.?

• 1- Since I'm a citizen my passport application has to be new or renew? 2- When ask for "book number" last passport number is not working but the one in my certificate. Does that mean that I have to put the information from my certificate? 3- Do I need to send original documents along with my application or copy? for example, birth certificate, prove of citizenship, etc. Thanks in advanced guys!

  1. You need to apply for the first U.S. passport using form DS-11 2. This form does not ask for a previous passport number. 3. You need to send the original of your naturalization certificate as proof of citizenship and ID; it will be returned to you by separate mail. A birth certificate is not required for those who have naturalized. Name changes must be supported by appropriate documentation (marriage certificate).
