How to refresh page on form submission?

Form/submission page help in PHP? much thanks! I'm still new to PHP?

• Willa anyone show me how to make this PHP form/submission page? 1) Create a form page and a submission page. On the form page, ask the user for his/her name, street address, city, state, and zip code. Using a select menu, have the user choose between a small, medium, or large soda, where small is $1.00, medium is $1.50, and large is $2.00. 2) On the submission page, collect all of the submitted information and output the user's name and complete address (in a readable format). Also output the total cost, which will include a sales tax of 5% and a shipping charge of $1.15 for small and medium sizes or $1.50 for large.

  Right this is actually really easy with a little research you could have this done in a couple of hour. And you may learn something. If you would have just asked it so it didn't read like homework you would have gotten better answers but I'm not up for being used by people who can't be bothered.
