How to make a mailing list for my site.?

I need to make a mailing list for a site I am working on. The only catch is that I need a way such that when the user is adding their email to the list, they list their username for the email and then choose an email provider (Google, Yahoo!, Microsoft etc.) from a drop down list. This has to be then translated into an email address and placed into the mailing list. For example, an input of "test" in the username field and a selection of "Yahoo!", should translate into [email protected] being added to the mailing list. Any ideas would be helpful. Thanks in advance! :D
