See site below for all passport application info and to print out the forms. No, you do not have to get vaccinations to get a passport. There are a few countries that require vaccinations, not many. When you decide to travel to a foreign county, go to the 2nd site below and pickthe country from the drop down menu and click go. then scroll down the page and it will tell you if there are any required or recommended vaccinations.
