As long as you are a Green Card holder residing in the States it depends on the passport of your home country whether you will need a visa or not. Some countries however grant U.S. Resident Permit holders a visa exempted tourist stay regardless of their citizenship. Those countries are: Aruba, Canada, Dominican Republic [get tourist card on arrival], Guatemala, Honduras, Jamaica, Mexico, Montenegro (7days), Netherlands Antilles, Nicaragua, Sint Marteen, Turks&Caicos Islands. A U.S. permanent resident cannot get a U.S. passport. Only American citizens can get U.S. Passports. You will need your passport from your home country to travel to any country except Canada and Mexico. a few countries will let you enter without a visa. For other countries , whether or not you need a visa , depends on your nationality. You will have to look it up for any country you want to travel to. Some countries require visas, some do not. If you do not have a current passport from your home country, you can get one at the embassy of your country in the U.S. You can use the site below to see if you need a visa or not to any certain country. Just fill in all the blanks and click for the info. If you do need a visa, you will need to look up the embassy of that country to the U.S. on-line and then read on that site to find how to apply for a visa.

Yes you do, although some countries, Philippines for one, will allow entry with out a visa for a limited time only, like the philippines will allow 21 days, if you want to stay longer, a visa is required

Just because you're a resident of the United States doesn't mean you're a resident of other countries. You obviously need Visa still, and a passport for the airport.
