Where is the document number on a passport card?

Document number on Canadian health card? (Passport application help?)?

  • It says to provide 2 documents to support my identity, and it needs a document NUMBER. My health card has a number on it which appears like this #### - ### - ### - TH, would the -TH be included in the document number? Thank you so much if you know!

    each province has their own health card. my province has no letters, just numbers. a phone call to your provincial medical plan would be in order, but i think it would be necessary for the whole thing, can't hurt.

The Quebec Health Card begins with 4 letters and they are treated like numbers for the Passport application so I would treat any letters as numbers.

It does include the letters. Calling it a document number is a throwback to older ID standards where they only used numbers, even though a lot of them include letters nowadays.
