Answer:
Are you going by Air? If so then the baby would require a Passport, it should also be 6 weeks old before it starts flying so that it does have a better immune system. If you are traveling by Land or Sea then the baby would require the birth certificate, nothing more than that. You yourself need a Passport, Passport ID card, NEXUS card to go by Air. If going by land or sea then the same or and Enhanced Driver's License. Anyway you are traveling if the mother is not with you then you will require a Power of Attorney to cross the border without the other parent. She will need one as well to travel without you.

First, since your baby was born in Canada, she is a Canadian citizen. All Canadian citizens may freely enter the U.S. without a visa and visit for up to 6 months a year. So, your girlfriend and the baby may certainly come visit in the U.S. Anyone 16 and over must have a passport. The baby may travel with just her birth certificate...the legal one from the government. Now...your baby is also eligible for American citizenship and you need to take care of this right away. You must go to the nearest American embassy or consulate and register the birth. This establishes and documents the baby's American citizenship and she will be eligible for an American passport. See 1st site below. Do not bring that very young baby to the U.S. without her mother!! And be sure to get an ok from the pediatrician for the baby to travel. If you want to live in Canada permanently and legally, your girlfriend must apply for a partner visa for you. See 2nd site. If you want her to come live in the U.S., you need to apply for a fiance visa for her. See 3rd site. Be very sure you register the birth of your daughter at the American consulate and establish her American citizenship so that when she is older she may live in either country...and she will have the benefits of American citizenship. See 4th site for list of American consulates in Canada. Congratulations on your baby girl! To travel by land from Canada to the U.S. , kids 15 and under need only their birth certificate. if you are going to fly, then the baby needs a passport. See 5th site.

She will need her own passport and a letter from the other parent saying it's ok to travel with her. Probably easier if you move grandma to Canada for a visit. She will also need passport.

Vic, it doesn't matter if the baby is one week or is 99, as of 2009, passports are required for boarder crossings. I would also carry a copy of the babies birth certificate.

You will need at the very least a letter signed by the mother authorizing you to take the baby with you.
