So the lower on the QALY scale, the earlier you stop spending. examples: Predict when it looks like benefits will start to level off (no further benefit from spending). Stop spending welll before then. Predict when there will be a point of inflexion in the spend curve (added vlue vs. spend will be linear). Stop spending well before then. This way you may end up spending the same money as if you used criteria such as "fair innings"/"intergeneration equity" but there's nothing sinister about what you are doing. There's a good case for economy of effort.

  The problem with your thinking is that you neglect teh legal aspects of an insurer's contract with the insured. Are you referring to a hypothetical Utopian community? Just wondering.
