You become a doctor by taking a pre-med course (which you take up after High School and for four years usually) then you go to med school. After four years in med school you're already a general doctor. But if you want to have specializations, like for example you want to be a Dermatologist or a surgeon, you'll have to extend med school. The number of years you'll have to extend depends on what specialization/s you're deciding to take (for example, Dermatology usually just adds three years).
