Medical question?

Just asking a medical question to students and practitioners in the medical field....?

  • It's just a "medical" game if you wanna call it like that. I ask a question in a simple medicine-related subject and you answer, then ask another question, and the one beneath you should answer your question and so on. Just for some additional knowledge. (I'm a med student) How would you diagnose this: (Commonest cause) A patient with a pin-point pupil, respiratory depression and decreased bowel movements (i.e: Constipation). And what antidote would give this patient?

  • Answer:

Pangolin: hows about a dose of rifampicin for leprosy? Next patient, a 13 y/o boy with a 6 month Hx of morning headaches. He presents with the first seizure in his life. Head CT scan showed small hyperdense areas on the frontal, right and left parietal regions. The kids favorite snack is steak sandwiches, which he likes medium-well. His concerned mother adds that he doesn't seem to be getting enough veggies so she once in a while picks fresh veggies from the farm they live on -- "he's just like his father and eats them raw," she states. _______________________________ D: No, but you're on the right track for my case (a Naegleria infection doesn't explain the CT scan); Hint -- the boy hates the smell of the neighboring pig farm. As for your case: cutaneous T-cell lymphoma? _______________________________ Ty: this has been fun!

Pseudotumor cerebri! Look for papilledema, check an MRI of the head, and do an LP, looking for elevated pressure. Next patient has a rash with numb areas on the skin, and a penchant for killing and eating wild animals of all sorts. He just got back from a camping trip in Texas. What drug will cure him? Ty: Something that's now rare but used to be quite common... think biblically.

Pangolin

OK, I'll play. Narcotic overdose; naloxone, though how you'd get the history of constipation is beyond me. What's the worry in a 25 year old chubby woman with vague headaches and blurred vision of several weeks' duration, and what to do to diagnose it?

Pangolin, is it leprosy? If so the treatment is rifampin and dapsone X 6mo. Aiden, is it Naegleria? If so, the trx is high dose Amphotericin B & rifampin. Next pt: 54yr old male presents to his GP with c/o psoriasis that has not responded to trx & seems to be getting worse. Close observation reveals several skin lesions that were once plaque-like have developed into ulcerations & soft tissue tumors. PE reveals lymphadenopathy, & HSM in addition to his skin lesions. Skin bx showed epidermotropism of lymphocytes & Pautreier's microabscesses.

Pin point pupils and rspitory depression means opiate/oid overdose or poisioning. There is no antidote but a large dose of narcon could displace the opiates from there receptor sites and bring the person back. withdrawals may occur in dependant users. If narcon is not availible then a shot to the heart of epinephrine may be necessary to revive the patient.
