How much does it cost to change your last name in New York State?

My dad is a douche and I decided to change my last name to my step dads name. I've been on the computer for more than 3 hours to find out how much and how long it will take. I live in New York State. I just wanna know how much it costs to change it. Where do you go to change it ( I live in Madison County ny), and is there a wait to change it, because my birthday is in 16 days, and I wanna change it before then. Thank you

I just googled "NY law name change" and this came up right away: http://www.courts.state.ny.us/courts/nyc/civil/int_minornamechange.shtml stating that it's $65 to change a minor's last name. However, you can't do it on your own.
