Can i cancel my passport ?

i am 21 years old, i have a MINOR passport and it expired on 1995 , i want to cancel this MINOR passport and need a new passport , is it possible ? the existing passport include so many problems such as ,name change, date of birth change and address change etc. So i want a new passport with correct datas, can i apply for a new passport without mention the existing one ?

If there is an address change then apply for a new one. Don't mention about the old passport that you had.
