What do you need to get a Canadian Passport?

What other documents do I need to get a new ohip card and passport?

• I can't seem to find the answer on the web I have a Social Insurance Number ( SIN ) I have a red paper health card with the number hand written on which I received when I moved to Canada in 1991, I was born in 1989 and am 20 years old. I was born in England as was my father, my mother was born in the USA, my father grew up in Canada and is a full citizen my father says I am a full citizen but the only paperwork we have is a Paper Red and white ohip card issued in September '91 bearing my name, ohip number, date and some other numbers and a SIN card with my full name on it which is made out of the proper card plastic. I need a New OHIP card with the picture and all and a Canadian passport, I need a citizenship card to get the passport. do I have enough to get the citizenship card or the ohip card, what other proof of identity/citizenship do I need? I also have a UK birth certificate and a US passport

  To be a Canadian citizen your birth in England would have needed to be registered at the Canadian High Commission or a Consulate General in the U.K. and it should have been issued a Certificate of Registration of Birth Abroad (RBA), which is the appropriate document to proof your Canadian citizenship for your passport application. http://www.pptc.gc.ca/cdn/section4.aspx?lang=eng ..
