How to fill out a form for an international student admission... I am confused and it is due to june 1st..?

ok, well I am from south america, and I've been talking to this college at the US, they sent me the admission papers, and I have to fill them out... My question is... My sponsor will be my dad, since he has a bank account in Miami, but he lives over here, in south America, so I am not sure how to use him... how to fill this... I also have my mom's best friend hse lives over there, and she has a good bank account which could help me... I just don't know... I need to know how to fill the I-134, Affidavit of support... Please someone HELP ME!!!!!!! I DON'T KNOW WHERE ELSE TO ASK. I must fill this form along with other forms, and send then to the college before june 1st, so they can send me the I-20
