What's a supplement worksheet for a passport?

When I turned in my passport application to the court house they said I would in a few weeks get back my birth cert. and get my passport. It's been about 3 or so weeks and I have received a supplemental worksheet. It says, unfortunately the identification I provided is not sufficient for passport purposes.. but I sent them with my passport everything I was suppose to send ( State ID card, since I don't have a license, SSC number, birth cert., and two photo's for my passport. Does everyone get this after sending in the passport app?? After I fill this out what am I suppose to do? Will I not get a passport or something?

Just send in the extra paperwork they they and your passport will follow.
