How do you get a sponsor sheet for race for life?

I signed up online to do race for life this year. I set up my online sponsor page but would also like a sheet that i can take round to neighbours etc. that can write their donations on that. I've tried numerous times to find out how to get a sponsor sheet on the website, but just keep getting directed to my online sponsor page. Its my first time doing race for life and I want to get the most sponsors possible. Anyone know how to get a sponsor sheet?

You can download the fundraising pack and print off a sponsor form. http://www.raceforlife.org/fundraise/fundraising-ideas.aspx?cat=0 If you use your mouse to scroll through there is an option to print, I have done so and a sponsor sheet is available. When you get to that page just go to file and print. I must say it’s not very organised to say the least!
