Have you checked with your employer? They may have it in their records. If that doesn't work the go to - http://www.sss.gov.ph/sss/index.html It is the official web site of the Philippine Social Security System. Here you can get a directory of branch offices and download SSS forms. To recover your SSS you may go to a branch office with proper proof of identification. If you are not near a branch you can try the SSS automated telephone service by calling the SSS IVRS at 917-7777 or call 920-6446 to 55 to speak to an SSS representative from 8:00 am to 5pm, Mondays to Fridays.
