How to get rid of rough dry patches on hands?

Really dry hands?

• I get these big patches of dry skin on my hands (at least I think that's what it is). They are barely visible when my hands are dry but as soon as I wash my hands or do dishes my hands become covered in this white splotchy stuff. I have used sandpaper to attempt to get it off...and some comes off but not all of it. I use lotion but it doesn't seem to do any good. What else can I do to get my hands back to normal?
