    I'm assuming your asking about why you get the GI problems first, then the rest of the typhoid symptoms. The salmonella bacteria cause the enterocolitis first as the invade the gut mucosal cells, thus you would see diarrhea/dysentery first. Afterwards, the bacteria enter the blood stream and disseminate throughout the body, causing the rest of the typhoid symptoms. This is actually important for diagnosis. For example, if you check for the bacteria in blood cultures while the initial stages of the illness, you won't find it because they haven't gotten to it yet. Same goes for the antibody test (Widal test), which comes out positive ~ 2wks after the initial inoculation. Typhoid fever usually includes the enterocoloitis symptoms so the entire illness is considered typhoid fever, with different organ systems being affected at different times of the illness.
