How to write medical report?

I want to know how to write a good report?

• I am a secretaria student and i have been send on an intenship for two months and this is the last month to finish my internship and i need to write a report after my internship. but i don't know how to write the report, please guide me on the neccesary points . give me the guide lines in other to write a good report.
