Whom/How to ask for a college recommendation letter?

How to ask a teacher to write me a college recommendation letter?

• after i ask my teachers i have to put it in a letter to that teacher so they will not forget. the letter has to be short but official. how should i start the letter? how should i word it? please and thanks!!
