Coverage characteristics of health insurance?

Where can I find info on NY Medicaid Health Insurance coverage?

• So the college I applied to makes you pay about $2000 a semester for health insurance by default. However I have health insurance already [Medicaid/Affinity] in the state of New York so I need to fill out an insurance waiver so I don't have to pay it. 1) My plan offers coverage of at least $100,000 per condition per policy year. 2) My plan covers inpatient and outpatient medical care within 25 miles of the University at Buffalo campus area. (If your plan has a referral or prior authorization requirement for local medical services, you agree to obtain and receive such authorization) Emergency Only coverage does not satisfy this requirement. 3) My plan covers inpatient and outpatient mental health care within 25 miles of the University at Buffalo campus area. (If your plan has a referral or prior authorization requirement for local medical services, you agree to obtain and receive such authorization) Emergency Only coverage does not satisfy this requirement.' 4) My plan provides coverage for pre-existing conditions or I have met the pre-existing waiting period of my plan. 5) My plan provides prescription drug coverage (either as part of my medical plan or as a separate prescription plan). 6) My plan is currently active and I agree to maintain health insurance coverage through the entire 2011/2012 academic year. 7) I will retain my other insurance coverage for the entire Spring period(January 12, 2012 through August 21, 2012). If my current plan ends earlier, I will either purchase the SMI or other equivalent insurance plan within 31 days. I also understand that I must supply a letter of creditable coverage from my insurance company for enrollment outside the open enrollment period. 8) I acknowledge that I am responsible for payment of all fees for medical and mental health treatment not covered by my medical insurance plan (including but not limited to deductibles, copays, coinsurance and the expenses above my policy maximums and benefit limits). I understand that some health facilities may require payment at the time treatment is provided. As you can see, there are questions on the application that I don't know the answers to, so can anyone please help me out here? I tried calling the numbers I found on the internet but they kept tossing me back and forth back and forth. I would really appreciate it any help.

  Don't you have a customer service number on the back of your insurance card? Just call them. Or call the school and ask them if they know whether or not Medicaid meets all their requirements. You can't be the only student they have who's covered by Medicaid.
