How can i print off a statement with my address?

I need a bank statement with my name and address on for this afternoon and my bank seem to have stopped sending them to me in the post even though i haven't asked them to do this. I look at my statement online and you can print it off but it doesn't seem to have my name or address on. Is there anyway i can get it online with that information or if i go into a branch will they print one of for me there. My bank is Natwest thankyou :)

If when you print off the statement, your address is not shown; I suggest you 'highlight' the whole page, right click your mouse and click on copy. Then open up a blank Word page, right click it and paste onto the page. Then you will be able to print off this page with your address on. Hope this is clear.
