Help Translating From Japanese To English?

I need help in translating from Japanese in English?

• Hi! So i have applied for exchange in Japan and I got the place but I still need to do some paperwork and I received the application form from the University and it is in Japanese. Unfortunately my Japanese is not that good and also I don't know anyone who knows Japanese.   I tried to translate the very first parts, phew. I didn't know most of the kanji myself, but after some research it was possible, at the very least. This is some good practice for me. I'll translate more later on, or maybe someone else wants to translate the rest. Anyway, I made a picture of the first part, I only translated the first box and the picture part: http://puu.sh/lsrS
