What message should I write in a card to a person with a fatal condition?

I want to write a message in a card to my husband's cousin who has a heart condition, and unless she gets a heart transplant, well, she sadly won't be with us anymore. What do you write in a card to someone in that situation? I can't really say 'Get well soon' or then add 'If you need any help give us a call' because they live too far away. Some advice would be much appreciated. Thank you xx
