How to write increment letter?

I need to write a salary increment letter?

• I have worke this company for the last ten months as a Costing Executive.During this time i have discharged my duties to the best of amy ablities.I have been sincearly dedicated and have constantly worked hard.I have been commited my job,and have sough to achive management's objectives. However , in my interview ,the interview pannel agreed me to the salary os Rs. 15000 for frist six month and thereafter my confirmation they agreed me to aroud Rs. 25000 salary.But still i have not received the above agreed salary.I am anticipating to know is there any problems with my works or behivour, not to upgrade my salary for the agreed level.so in this case i am facing lot of defficulties. Further i travel here daily by bus and have to spend aroud Rs 3000 per month as my bus fear. I am a graduate and part qualified of charted Accountancy and I have 2 year worked experience also. so I kindly request you to grant me your kind attention to slove my problem by considering the above matters
