It is a management system that is designed to anticipate and prevent problems. The idea is that you document as many processes as possible and try to assess the risk of things going wrong and then devise counter systems to prevent errors. An example might be say Ford, designing the lights on a car in such a way that the guys on the production line cant put them in the wrong way round. The reason they might do this is because if it possible to do something wrong, then someone will do it - guaranteed! Of course when you try to make something idiot proof then "management" usually discovers that the "idiots" are far more inventive than they have been given credit for.
