I have to write a personal statement. Any help?

I need help to write a statement to my boss?

I have a company cell phone. My personal cell phone was disconnected so i was receriveing text messages on my work phone. I was called in to my bosses office and was told that 100+ dollars were charged because of the text messaging. I offered to reimburse the company, they dont want the reinbursment just a statement from me explaining the situation and not to doit again. I just dont know how to write one. (I have my personal phone back)
