How to write a forwarding letter?

I have to write my own letter of recommendation but what do I write?

• My boss is willing to "write" a letter of recommendation for me for medical school, but since his english is sketchy at best, wants ME to write the letter, go over it with him, and then he will sign his name and submitt the letter. I have absolutely NO IDEA what to write here, and I'm not all that excited about writting a letter about how awesome I am for someone else to sign their name on. Help?
