How do I write a letter to my daughters school requesting holiday leave?

we are meant to be going on holiday in july for a family wedding. this means my daughter will miss the last week of school. she goes to a private school and has a scholarship so it is important to make the letter quite formal. How can i write this letter.

Read the terms of the scholarship first and make sure you aren't jeopardizing her next year (or this one for that matter) You may need to leave her with relatives/friends to finish up the year and join you after school is out. If there is no gate to attendance then simply write " X will not be attending school from X to X. She will be attending a function with her family. Please advise us how we can arrange for any schoolwork missed during this time to be done during the holiday or directly after."
