How to write a study plan?

What is in a good homework, assignment and study plan?

• How much time should I dedicate to my homework if I have five, 1 hour periods on Monday, Tuesday, Wednesday and Friday? (On Thursday I have three 1 hour periods). How much should I do of my assignment per day (or per every 2 days) if I usually have 2 weeks to complete the assignment and hand it in? How much time should I dedicate to my assignment/s? How long should my breaks be and how often should I have them? I plan to study on the weekends only. How much time should I dedicate to my studying for all my school subjects? What is the most effective way to study? The way I study at the moment is to write down summary notes for the topic I am doing, I do this for all my school subjects. Then my studying is just reading over the notes. What are some other fun, but effective ways to study? How do I fit in my television and dinner times? What would you suggest I do if I have really long television shows that is longer than my study times? What would you suggest I do if I have really long television shows that are longer than my study times? I also have my assignments at random times and my homework varies each day.
