How to apply singing exercises when actually singing?

I've learned many different vocal exercises/warmups, such as lip trills, humming, singing mum, etc etc etc... I can do these exercises fine, but when I sing an actual song, my voice falls apart. Help!

Answer:
You should get a voice coach. They can listen to what you're doing and point out whats wrong with what you're doing. It's helpful because, as singers, we focus on the emotion behind the lyrics and the connection between our voices and the music but can't pin down what we're doing wrong. Vocal coaches are VERY helpful! :)
