If you decide to donate your eyes contact an organ donor association and fill up the organ donor card and follow further instructions. After your death your next of kin must inform authorities as early as possible as eyes can be collected upto 4-6hrs only. Also even if you give consent before death, to donate eyes your next of kin must give consent to organ donation after your death, else your organs can't be collected.
