What does plano, OD, +25, 90, +2.50 mean on an eyeglass prescription?

I have a prescription for reading glasses and want to get clear on top and reading on the bottom. The prescription reads OD - plano - 90 - +2.50 & OS plano - blank - blank - +2.50. Will this do what I want.

those are all lens "powers" or more precisely focal lengths of lenses in "diopters" or reciprocal meters. OD is right eye, OS is left eye. ADD is the "bottom" reading portion. the doc (ophthalmologist, apparently since it's in "plus" cyl form) wrote you basically nothing on top & +2.50 diopters on bottom for reading. they did add in 1 click (1/4 of 1 diopter or +0.25) of astigmatism in the right eye only.
