Is it possible to identify someone whilst they are anonymous on 4chan?

I was on 4chan a while back and someone post a message along the lines of "hey [my name], you've not been on 4chan for a while". I'll be honest, at first it freaked me out a bit but I assumed it was just an amazing coincidence that someone was spamming using my name, and that I hadn't be on 4chan for a while. But then I started thinking - could someone with moderator/administrative powers identify me from my posts or images uploaded? Can anyone give me an insight into what information is available to the mods?
