Herniated disc in cervical?

Why is my herniated disc at l4-l5 worse at night and what can I do?

  • I have had a L4-L5 herniated disc for 9 months now. I have had 3 sets of MRI's for diagnosis purposes. 1 opinion from a local neurosurgeon indicated that there might be fragmented disc on the nerve. A 2nd opinion indicated that there was nothing more than a herniated disc. I have had 3 cortisone injections now. Throughout this entire process, I am almost pain free in the morning and practically crippled by 5pm. Every day I think this is a better day and every night I think I'm ready for surgery. Scared of the risks of surgery (that I will have pain throughout the day), I have chosen to fore go surgery but am now seriously considering it. What can I do and why is this pain so much worse in the evening?

Firstly, I'm not a doctor. Have you discussed how the pain gets worse with your doctors? Have they suggested putting a pillow between your knees when sleeping on your side? You can also put one under your knees, if you lay on your back. Adjust the pillow until you're comfortable. I'm guessing you've tried ice packs and heat. Ask to see a good physical therapist. They'll be able to help with exercises and information on how you can do things to keep your back from getting so painful; i.e.; how to sit, walk, stand, lie down, etc. properly for your condition. What about a back brace? How about a TENS unit you can use at home? If I were you, I'd get at least one (or two) opinions from neurologists/orthopedists - not surgeons - before you get surgery. Imho, surgeons tend to want to do surgery to help someone. The "ists" aren't so quick to recommend surgery. They have other options. If you decide on surgery, check your surgeon out very carefully. Make sure they're Board Certified - go to the link below to check on them. Ask to speak to some of their recent patients that have had the surgery they want you to have. Ask the doctor the hard but simple questions like- How many of these surgeries have you done? What is your success rate on reducing the patient's pain? Think of your questions, write them down and ask the doctor. Take a small tape recorder (ask the doc - I've never had one say no) to record the answers and recommendations. Research the procedure, too. Bring any information you have questions on, to your doctor appointments. In other words, be a very well informed patient. Whatever you choose to do, I hope it helps you to feel better.

It is the strains of moving around in the daytime that build and tire you out. Skeletal problems all feel best after rest. Everybody I know that has had the surgery has less pain afterwords than they had before, except that after surgery it is constant pain, day and night. So patients accommodate to it, but it is a last resort. It is good that you have some situations that are without pain for you. There are a number of alternative therapies that you can try before finally having to opt for surgery. They range from chiropractic and acupuncture to exercises and herbal remedies. Many, many people have your condition and are able to live pretty normal lives without surgery.
