Question About Medical Consent?

• I am going on a road trip with one of my best friends. We will be gone for a little over two weeks and plan on visiting many different places. We thought that since we are visiting places we haven't been before and dealing with new and different areas, landscapes, climates... etc and will be very far from home that we should have some kind of medical releases for each other just in case something happens. We are both 21, so we can legally make our own decisions concerning our medical care, however, just in case something happened that left either of us unable to make our own decisions (i.e. loss of consciousness) we want to be able to make decisions for each other. Is there any kind of medical release/consent form that deals with giving authorization of medical decisions to someone when you are over 18? I have looked through results on 5 or 6 different search engines and can only find medical forms for parents giving permission to other adults in regard to their children (like, for a field trip). If there isn't a form, can we make up our own and sign them? And if so, what information would be important to include on our form? Thanks so much

  You need either (or all) 1.An Advance Directive. It states what measure you would approve of for life sustaining or life saving. 2.Medical Power of Attorney. Grants authority to the person named. You should be able to find and download both on the internet for free or at Legal Zoom.com for a fee.
