How can I learn the true cause of my father's death in a local hospital where family was told very little?

Hospital officials told my mom, sister and me very little about the cause of my dad's unexpected, sudden death as we waited to visit him in ICU. Days before he died, my dad was scheduled to be transferred from ICU to a regular room because he had improved so much. Apparently, he took a turn for the worse. On the day Dad died, we were told that hospital personnel worked on him and "lost his heart rate." The death certificate cause of death is listed as "pneumonia." Family members never were told that my dad had pneumonia or died from pneumonia. Was this a conspiracy of silence to conceal an accidental or preventable death due to negligence? We want to know.
