Where can i get a copy of son's shot record?

  • My 3 yr old is starting daycare and they need his shot record. The problem is that his doctor is no longer at the same location and is not listed. Where else can i get a copy of the shot records?

Call your son's pediatrician's office- even if the doctor that saw him is no longer at that office, they should have your son's records or can give you contact information for who does have the records.

Is just the doctor gone or the entire office? If just the doctor is gone, the office will still have your sons records. If the entire office is gone, call the Health Board and let them know the situation. They may be able to help you out.

Dr's offices have to report all immunizations to the state. call your local health nurse adn ask how to get them she will be happy to help you.
