Can you add a sql database to an existing bluehost account?

Can I create a sql database and username in the control panel of my existing bluehost account for free, so that I can create my wordpress blog? Or do I have to purchase another url/domain name in order to host my own blog with wordpress?

  No, you should be able to create a database. You could install WordPress in a folder off your main domain like youdomain.com/blog or yourdomain.com/wordpress and use it off your main domain. Then it won't affect your main website. You can add a links from your main site to the blog so people visiting your site can then find your blog.
