How to get a C.L.U.E. report?

My sidekick slide was stolen and i have insurence how do i get a new one?

    You should claim insurance here is number 18662687221 you can also cal T-mobile customer care 18009378997 its might deductible to claim 50 to 75$ but they will send you brand new phone. don't lie to them they will not process your request.

you first go(or call) ur local police station and file a report about a stolen phone, then you call T-Mobile and T-Mobile is gonna ask u for a police report number and den they're gonna tell u wat to do. and i think u gonna have to pay a deductible. good luck. :)
