How to sign up for Double Day Book Club.ca (Canada)?

Hi, I used to have an account not more then two weeks ago, but now I forgot it and I don't know how to create an account, I do but it doesn't work there is no: Register, Join now, Sign up or w-e button.! So, can you please help me?!:P

I got to it by: 1. Going to http://www.doubledaybookclub.ca/ 2. Clicking the "Get 5 books for 20 cents each" text in the scroll on the leftside of the page.
